By creating an account or using the PracticeSentry platform ("Service"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree, do not use the Service. These Terms apply to all users, including administrators, employees who self-enroll, and any organization accessing the Service on behalf of its workforce.
PracticeSentry provides credential monitoring and compliance automation tools for regulated businesses. This includes verification of employee licenses against official government sources, exclusion screening against databases such as the HHS-OIG List of Excluded Individuals and Entities (LEIE) and SAM.gov, expiration alerting, compliance reporting, and related features.
PracticeSentry uses automated systems, including AI-assisted tools, to perform verifications, detect risks, and generate reports. The field of artificial intelligence is advancing rapidly, and while we make every reasonable effort to provide accurate and reliable results, we cannot guarantee that all automated outputs will be complete or error-free at all times. We are committed to continuous improvement and will do our best to maintain accuracy as the technology evolves.
PracticeSentry is a compliance monitoring tool, not a legal or regulatory advisor. Results provided by the Service are informational and should be reviewed by a qualified compliance or legal professional before being used as the sole basis for employment, credentialing, or regulatory decisions.

Paid plans are billed monthly or annually as selected at checkout. Prices are listed on our Pricing page. All fees are non-refundable except where required by law or as stated in our refund policy.
We reserve the right to change pricing with 30 days' notice. Continued use of the Service after a price change takes effect constitutes acceptance of the new pricing. Payment processing is handled by Stripe. By providing payment information, you authorize us to charge your payment method on the billing cycle selected.
Your use of the Service is also governed by our Privacy Policy, which is incorporated into these Terms by reference. By using the Service, you consent to the collection, use, and processing of data as described in the Privacy Policy.
THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTY OF ANY KIND, EXPRESS OR IMPLIED. WE DO NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED, ERROR-FREE, OR THAT VERIFICATION RESULTS WILL BE COMPLETE, ACCURATE, OR UP TO DATE AT ALL TIMES. YOUR USE OF THE SERVICE IS AT YOUR OWN RISK.
TO THE FULLEST EXTENT PERMITTED BY LAW, PRACTICESENTRY AND ITS OFFICERS, EMPLOYEES, AND AFFILIATES SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF OR INABILITY TO USE THE SERVICE, INCLUDING BUT NOT LIMITED TO REGULATORY FINES, PENALTIES, EMPLOYMENT ACTIONS, OR BILLING DISPUTES, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. OUR TOTAL LIABILITY TO YOU SHALL NOT EXCEED THE AMOUNT YOU PAID FOR THE SERVICE IN THE THREE MONTHS PRECEDING THE CLAIM.
You may cancel your account at any time through your account settings or by contacting us. We reserve the right to suspend or terminate your access if you violate these Terms, engage in fraudulent activity, or if required by law. Upon termination, your access to the Service ceases and your data will be deleted per our retention policy.
We may update these Terms from time to time. Material changes will be communicated via email or in-app notice at least 14 days before taking effect. Continued use of the Service after that date constitutes your acceptance of the revised Terms.
These Terms are governed by the laws of the State of Ohio, without regard to its conflict of law principles. Any disputes shall be resolved in the courts of competent jurisdiction in Ohio.
